Here are mandatory requirement for elementary school enrollment:

 
1.   5 shots of DTAP (the last shot at age 5) --- 白喉、破傷風、非細胞型百日咳混合疫苗（DTaP）

          OK （六月個大完成第三劑，一歲六個月大追加一劑新型非細胞形三合一疫苗，一年級打第五劑）

2.   4 shots of Polio (the last shot at age 5) --- 小兒麻痺口服疫苗 

         OK  （六月個大時完成第三劑，一歲六個月大追加一劑，一年級打第四劑。）

3.   2 shots of MMR (last shot at age 5) ----麻疹、腮腺炎、德國麻疹混合疫苗(MMR) 

          OK （一歲三個月大，一年級下完成第二劑）

4.    4 shots of HIB (Haemophilus influenzae type b), last shot at age 3  --- 乙型流感嗜血桿菌(Hib) 疫苗， 

           OK，兩歲半時完成第四劑。

5.     Varicella or history of chickenpox  水痘疫苗， 

             OK， 兩歲三個月施打一劑。

6.     3 shots of hepatitis B  B型肝炎，

              OK  （六月個大時完成第三劑）
 
It will be easiest to get those shots at Taiwan if Journey hasn't gotten it.  However, he can always have them when he arrives.

Journey 以上疫苗皆已施打完成。

申請前往國外就讀申請英文版接種證明書，可向戶籍地衛生所辦理。 需準備資料包括：

1.  接種記錄卡。

2. 護照。

3. 戶口名簿影本。

4. 申請費用。
